REQUESTING ACCESS TO, OR COPIES OF, YOUR MEDICAL RECORDS

You may ask to view your records, obtain personal copies, or direct us to send them to another person or organization (e.g., another provider, attorney, or insurer). Federal law gives us up to 30 days to respond, but our average turnaround is less than one week.

How to submit a request
  1. Complete an Authorization Form.
  2. Return the form via secure email (contact@nmph.org) or in person at any clinic location.
